The M.2 SSD preinstalled in a Lenovo Thinkpad P50. Strangely, I could not find the exact model number, but almost mathing ones are present in https://toshiba.semicon-storage.com/content/dam/toshiba-ss/asia-pacific/docs/product/storage/archive/cSSD/cSSD_HG6_Series_SED.pdf
